Terms and Conditions – SlimTherapy Partner Clinic Network

1. Program Description

SlimTherapy offers a collaboration program for clinics (“Partner Clinic Network”), through which independent medical centers may provide application and follow-up services for weight management treatments marketed by SlimTherapy.

 

SlimTherapy is responsible for customer acquisition, marketing, sales, and payment processing for the treatments.

 

Partner clinics are responsible for administering the treatment, providing patient care, and ensuring proper medical follow-up.

 

2. Application Process

To become part of the Partner Clinic Network, interested clinics must:

• Complete the application form available at: https://www.slimtherapyglp1.com/clinicas

• Provide truthful, accurate, and up-to-date information

 

SlimTherapy will review each application and reserves the right to:

• Approve or reject any application at its sole discretion

• Request additional information or supporting documentation

 

Once approved, the clinic will be added to the network and may begin receiving patients.

 

3. Nature of the Relationship

The relationship between SlimTherapy and the partner clinic is that of independent parties.

The clinic:

• Is not an employee, agent, or legal representative of SlimTherapy

• Operates under its own legal entity, licenses, and local regulations

• Is responsible for complying with all applicable legal, tax, and healthcare obligations

 

Nothing in this agreement shall be interpreted as:

• An employment relationship

• A partnership or joint venture

• An exclusive representation

 

4. Revenue Structure

For each treatment sold by SlimTherapy and assigned to a partner clinic:

• The clinic will receive 55% of the net sale value after deducting an operational fee, which may vary

• SlimTherapy will retain the remaining 45%

 

Conditions:

• Calculations are based only on successfully completed and paid transactions

• No payments will be generated from canceled, refunded, or disputed transactions

• There is no guarantee of minimum patient volume or income

 

5. Payments

Commissions payable to the clinic:

• Will be settled on a biweekly basis

• Will be paid using the payment method selected by the clinic during registration

 

The clinic is responsible for:

• Providing accurate payment information

• Covering any associated banking, tax, or administrative costs

SlimTherapy is not responsible for errors resulting from incorrect information provided by the clinic.

 

6. Clinic Responsibilities

The partner clinic agrees to:

• Administer treatments in accordance with medical and regulatory standards

• Ensure that patients are evaluated by licensed healthcare professionals

• Provide appropriate medical follow-up during treatment

• Comply with all applicable local healthcare regulations

 

The clinic is solely responsible for:

• Medical decisions

• Patient care

• Any outcomes or effects resulting from the treatment

 

7. SlimTherapy Responsibilities

SlimTherapy is responsible for:

• Customer acquisition

• Marketing of treatments

• Managing the purchase and payment process

• Assigning patients to partner clinics

 

SlimTherapy does not directly provide medical services within partner clinics.

 

8. Medical Disclaimer

SlimTherapy does not guarantee specific results.

All treatments:

• Must be administered under medical supervision

• Require prior evaluation by licensed healthcare professionals

 

The clinic agrees not to:

• Promise specific results to patients

• Omit necessary medical evaluations

 

9. Limitation of Liability

SlimTherapy shall not be liable for:

• Acts, omissions, or medical decisions made by the clinic

• Claims, damages, or losses arising from patient care

• Regulatory non-compliance by the clinic

 

The clinic agrees to indemnify and hold SlimTherapy harmless from any third-party claims arising from its operations.

 

10. Termination

SlimTherapy may terminate the relationship at any time, with or without cause, particularly in cases of:

• Breach of these terms

• Inadequate medical practices

• False or misleading information

 

In the event of termination:

• The assignment of new patients will be suspended

• Only payments corresponding to valid, non-disputed sales will be settled

 

11. Modifications

SlimTherapy reserves the right to modify these Terms and Conditions at any time.

Continued participation in the program constitutes acceptance of the updated terms.
